Services

Community Support Program (CSP)  provides intensive care management using the ‘Assertive Community Treatment’ model which features; low staff to consumer ratios, services delivered in the community, and consumer advocacy and empowerment.  Eligible consumers are Brown County residents referred to Villa Hope by Brown County Health and Human Services.  Care managers meet with consumers as needed, up to several contacts per day, including weekends and holidays.  Emphasis is placed on successful independent community living and sound symptom management.  Consumers are adults diagnosed with severe and persistent mental illness.

Community Based Residential Facility (CBRF)  a licensed 15 bed Class A Semi-ambulatory group home serving adult men diagnosed with severe and persistent mental illness, developmental disabilities, traumatic brain injury, and substance abuse.  Provides a full range of supportive services including; nutritional meal program, activity program, medication supervision, care coordination, transportation to medical, dental, vocational and community activities.  Independent living skills such as budgeting, grooming and hygiene, use of public transportation and laundry care are taught/supported.  Residents are referred from all across the State of Wisconsin, primarily from Northeast Wisconsin.

VH APPLETON 1

 

Supported Apartment Program (SAP supported apartment living in the Appleton community.  Assists residents to maintain independent community living to the resident’s fullest potential.  Serving adult men diagnosed with severe and persistent mental illness, developmental disabilities, traumatic brain injury, and substance abuse.  Provides a full range of supportive services including; nutritional meal program, activity program, medication supervision, care coordination, transportation to medical, dental, vocational and community activities.  Independent living skills such as budgeting, cooking, socialization skills, vocational, grooming and hygiene, use of public transportation and laundry care are taught/supported.  Residents are referred from all across the State of Wisconsin, primarily from Northeast Wisconsin.  Consists of 8 homes in Appleton converted to upper/lower apartments, each having one to two bedrooms per flat.  Each unit consists of a private bedroom, shared living room, kitchen, and bathroom, and laundry facilities.

Special Apartment Program provides one-to-one support and supervision for adults diagnosed with severe and persistent mental illness, developmental disabilities, traumatic brain injury, and substance abuse.  The small staff of live-in caregivers provides consistent, compassionate 24/7 care to insure safety, security, and community involvement to the resident’s fullest potential.  Provides a full range of supportive services including; nutritional meal program, activity program, medication supervision, care coordination, transportation to medical, dental, vocational and community activities.  Residents are referred from all across the State of Wisconsin, primarily from Northeast Wisconsin.  The current program is located in a duplex in Kaukauna and serves 2 individuals.
