Community Support Program (CSP).
The Villa Hope Community Support Program provides intensive care management to consumers with severe and persistent mental illness. The CSP utilizes principles of the ‘Assertive Community Treatment’ (ACT) model to provide comprehensive, community-based service. The CSP features low staff to consumer ratios, services delivered in the community, and consumer advocacy and empowerment. Care is coordinated through a case management system with one identified staff as the single point of contact to each CSP consumer. Within this system, the CSP staff work as a team with the consumer to develop an individualized treatment plan that supports the consumer. This enables each individual to achieve their maximum potential for independence.
Emphasis is placed on successful independent community living and sound symptom management. Staff meet with consumers as needed, up to several contacts per day, including weekends and holidays.
The Community Support Program sponsors monthly social gatherings for interested consumers. A yearly recognition banquet honors consumers for their success in managing their symptoms and illness.
Eligible consumers are Brown County residents referred to the Villa Hope CSP by Brown County Human Services.
