Special Apartment Program.
The Villa Hope Special Apartment Program provides one-to-one support and supervision for adults diagnosed with severe and persistent mental illness, developmental disabilities, traumatic brain injury, and substance abuse. The small team of live-in caregivers provides consistent, compassionate 24/7 care to insure safety, security, and community involvement to the resident’s fullest potential.
A full range of supportive services are provided, including:
- Nutritional meal program
- Activity program
- Medication supervision
- Care coordination
- Transportation to medical, dental, vocational and community activities
- 1:1 supervision to insure safety
- Life skills coaching
- Symptom management
Residents are referred from all across the State of Wisconsin, primarily from Northeast Wisconsin. The current 3 programs are located in Kaukauna and Wrightstown, and the programs serve 7 individuals. The programs are located in residential homes in quiet suburban areas. Programs are tailored to meet the unique needs of each individual.
