Special Apartment Program.
The Villa Hope Special Apartment Program provides one-to-one support and supervision for adults diagnosed with severe and persistent mental illness, developmental disabilities, traumatic brain injury, and substance abuse. The small team of live-in caregivers provides consistent, compassionate 24/7 care to insure safety, security, and community involvement to the resident’s fullest potential.
A full range of supportive services are provided, including:
- Nutritional meal program
- Activity program
- Medication supervision
- Care coordination
- Transportation to medical, dental, vocational and community activities
- 1:1 supervision to insure safety
- Life skills coaching
- Symptom management
Residents are referred from all across the State of Wisconsin, primarily from Northeast Wisconsin. The current five programs are located in Kaukauna and Wrightstown, and the programs serve 12 individuals. The programs are located in residential homes in quiet suburban areas. Programs are tailored to meet the unique needs of each individual.
